The great city referred to here is the literal city of Babylon - seat of the apostate religious system which the woman, Babylon the Great, the mother of harlots, and abominations of the earth symbolizes.
The woman and Babylon are two facets of the same city and power: They will both make nations drunk with the wine of their fornications, and be responsible for the death of great multitudes of believers during the tribulation (cp Rev 6:9-11; 7:9-14; 17:1-6 with Re 18:2-3, 7, 9, 24).
